levodope as co careldopa or co benldopa, ropinirole, pramipexol

moa for dopa drugs
A
- In PD - nigostriatal pathway has deficiency of dopamine. this reduces excitation in motor cortex = sx of PD.
levodopa = precursor dopamine
ropinirole and pramipexol = selective d2r which are in striatum

SE PD dopa drugs
A
nausea, drowsy, confused, hallucinations and hypotension.
wears off - can be overcome by increasing dose and frequency but can worsen and have opposite effect = dyskinesias = on off effect

caution use of dopa drugs in who
A
elderly, cognitive or psych disease due to risk of hallucination and confusion, CVD as risk of hypotension

- Dopimanergic not with first gen antipsych and some second gen. or metoclopramide as oppose dopa effects

why should you not stop dopaminergic drugs abruptly
A
cause Neuroleptic malignant syndrome
