MOA PDE5I
A
- Inhibits PDE5 which is in SM of corpus cavernosum and arteries of the lung.
in the penis; for erection, NO released, which causes CGMP to be produced and cause vasodilatation and engorgement.
PDE5 normally breaks down CGMP. so inhibit = increased conc
only with sexual stimulation
in lungs - pulm vasodilation similarly

A
- Flushing, headache, dizzy, nasal congestion
- hypotension, tachycardia and palpitations and small icnreased risk of vascular events
- priapism - erection fails to subside
- visual disorders if PDE6 is inhibited in retina

- Stroke
- ACS
- CVD
- if taking other drugs that increase NO (nitrates or nicorandil)

avoid Sildenafil in who
A
- Hepatic or renal disease. conc of sild increases by cyp450 inhibitors
- already taking vasodilators; Alpha block (dont take in 4hrs of silden) and CCB
