MOA ARB

A
  • ARB block action of ang II on AT1R. = reduce peripheral resistance (afterload), lowers BP. dialtes efferent glomerualr arteriole = reduces intraglomerular pressure and slows progression of CKD.
  • reduce aldosterone = na and h20 excretion. = help to reduce venous return (preload)

when to stop ARB

A

In the absence of other causes, the ARB should be stopped if the serum creatinine concentration rises more than 30% or the eGFR falls more than 25%. If serum potassium rises above 5.0 mmol/L, stop other potassium-elevating and nephrotoxic drugs (see Important interactions). If, despite this, it remains above 5.0 mmol/L, reduce the dose of the ARB. If it exceeds 6.0 mmol/L, stop the ARB and seek expert advice.
