indications for adrenaline
A
- cardiac arrest; part of ALS
- anaphylaxis
- prolong local anaesthesia; causes local vasoconstriction e.g. in controlling mucosal bleeding

MOA of adrenaline
A
agonist of alpha1,2 and beta 1,2. = stimulates sympathetic nervous system. = vasoconstriction, bronchodilataion

SE adrenaline
A
- after restoration of flow in cardiac arrest = adrenaline induced hypertension
in conscious patients for anaphylaxis;
- anxiety
- tremor
- headache
- palpitaitons
if pre existing heart diseease; MI, Angina, arrhythmias

CI to adrenaline
A
- none in cardiac arrest and anaphylaxis
- if local vasconstriction use; caution in heart disease patients.
- do not use combo adrenalien anesthetics preparations in areas supplied by an end artery. e.g. fingers and toes - necrosis due to constriction

interactions of adrenaline
A
- beta blocker; combined can cause widespread constriction

when do you give adrenaline in cardiac arrest
A
- pre filled syrince = 1:10000 (1mg in 10ml) then flush.
- in VF or pulseless ventricular tachycardia. 1mg IV after 3rd shock and repeat every 3-5 minutes after.
- in asystole or pulseless akak non shockable - give as soon as IV access - adrenaline 1mg IC and repeat every 5mints

dose of adrenaline in anaphylaxis
A
- depends if adult or kid but adult adrenaline 500mcg IM repeated after 5mins if need be. NOT IV unless cardiac arrest too.
dose - 0.5ml of 1:1000 (1mg in 1ml)

dose of adrenaline in local cosntriction
A
ready mixed anaesthetic and adrenaline prep - adrenaline at conc of 1:200000 (5mcg/ml)
