A
- leg cramps due to sudeen painful contraction of skeletal muscle.
- quinine reduces excitability of motor end plate to ach
- in malria - kills parasite at schizont stage

SE quinine
A
- tinnitus
- deafness
- blindness
- GI upset
- hypersensitivity
- prolonged QT-i
- hypoglycaemia in malaria

CI quinine
A
caution use in existing hearing or visual loss
- teratogen in first trimester - although can prescribe if malaria
- G6PD; preicpitates haemolysis

interactions of quinine
A
do not prescribe with drugs that also prolong QT-i or cause aarhytmias e.g. amiodarone, antipsychptics, quinolones, macrolides and SSRIs

dose of quinine for nocturnal leg cramps
A
200-300mg take at night

how should IV quinine be given
A
slow infusion

how long should you prescribe quinine for leg cramps
A
4 week trial and reduce if better. if not improved by 4 weeks unlikely to benefit
