antivirals Indications
A
- rx of acute herpesvirus infections including herpes simplex e.g. cold sores, genital ulcers, encephalitis. and VZV
- Supression of recurrent herpes simplex attacks where occuring at frequency of 6 + / year

antivirals MOA
A
- herpes virus = DNA virus. this needs Herpes specific DNA polymerase for virus to replicate.
- inhibits herpes specific DNA polymerase

SE antivirals
A
- headaches
- dizziness
- GI disturbances
- skin rash
IV - phlebitis. it is very water insoluble so at high conc can precipitate in renal tubules = AKI. reduce risk by good hydration and slowing infusion

relative CI aciclovir
A
- pregnancy
- breastfeed but if severe infections must give
- dose reduction in severe renal impairment

aciclovir oral and genital herpes dose and mode
A
- topical 5x/day or 200mg five times a day. start in prodromal phase.
- recurrent infections = supressive treatmnet (400ng 12hrly)

- 10-15ml/kg 8hrly IV
- 14-21 days
