What causes a neuropathic ulcer?
A
- Combo of peripheral neuropathy, atherosclerotic changes, and pressure
- Often present in diabetic population

A
Epidermis only

A
Through epidermis, partly through dermis

Full thickness ulceration
A
Through epidermis, dermis, and into deeper layers like subcutaneous fat

A
- DM
- Peripheral neuropathy

Clinical presentation for neuropathic ulcer
A
- Pressure during normal WB
- Common sites: heel, toe tip, tip and dorsal aspect of hammer toe, plantar surface of metatarsal heads, and bunions
- Loss of protective sensation

Wound bed of neuropathic ulcer
A
- Well-defined oval shape surrounded by rim of hypertrophic callus
- Granular tissue present w/ little necrosis
- Minimal exudate
- Minimal pain
- Surrounding intact skin is shiny, dry, and inelastic
- Distal pulses diminished or absent

A
Slow to heal due to systemic impact of underlying conditions

A
- Manage contributing factors (blood glucose, hypertension, hyperlipidemia, obesity, atherosclerosis, renal insufficiency) through lifestyle modifications
