Gut organisms invade the appendix wall after lumen obstruction by lymphoid hyperplasia, faecolith (dehydration) or filarial worms. This leads to oedema, ischaemic necrosis and perforation. Hygiene hypothesis may cause impaired ability to prevent invasion.
