describe the pathology
pancreatic enzymes are released and activated, and they auto digest the pancreas
stage 1
oedema and fluid shift cause hypovolaemic shock, and fluid and enzymes in the peritneoa cavity autodigest the fat affecting Ca binding causing hypocalcaemia
stage 2
autodigestion of blood vessels causes retroperitoneal haemorrhage
stage 3
infarction due to comprmised blood supply causes pancreatic necrosis
necrotic tissue becomes infacted causing abscess formation
clinical features PAN
diffuse pain radiating to back - relieved by sitting up
anorexia nausea and vomiting
and erythema abigne
how can severe acute pancreatitis present
like peritonitis with widespread guarding and absent BS
flank brusing
grey turner’s sign
periumbilical bruising
cullen’s sign
cardiac like symptoms
hypotension and tachycardia
reason for hypotension and tachycardia
in hypovolaemic shock due to cerebral oedema and fluid shift
general symtoms
abdominal pain loss of appetite - weight loss temperature jaundice
causes
Gallstone Ethanol Trauma Scorpion or spider Mumps Autoimmune Steroid Hyper lipidaemia/calcaemia ERCP Drugs
diagnosis - amylase lipase

US assessment of biliary tree
standrad imaging for assessing severity and complications
