Outline the pathogenesis of angiodysplasia

A
  • Mostly in the caecum and ascending colon
  • Probably a combination of multiple factors
    • Age-related degeneration of vessel walls
    • AV shunting from chronic obstruction of submucosal veins due to repeated episodes of high intraluminal pressures

  • End-stage renal disease
  • Von-Willebrand disease

What are the principles of management of aniodysplasia?

A
  • Active haemorrhage
    • Endoscopy, then mesenteric angiogram, leading to colonoscopy and adrenaline/cautery/resection
  • Haemodynamically stable
    • Endoscopy, angiogram, surgery if necessary
