Class / Action
A
Vasodilator
- Dilates Veins and Arteries in Peripheral Circulation Resulting in:
a. Reduced resistance to blood flow
b. Decreased blood pressure
c. Decreased workload on heart - Dilated coronary arteries
- Dilates blood vessels in smooth muscle; (i.e. GI tract, gallbladder, bile ducts, uterus)
- Improves cardiac output in patients with CHF

Indications
A
- Chest Pain
2. CHF with pulmonary edema and adequate BP

Contraindications
A
- Known Hypersensitivity
- Systolic BP<100
- Use of erectile dysfunction drugs or pulmonary hypertension drugs, such as sildenafil or tadalafil, within 48hours

Precautions
A
- When HR <50 or >100
2. With evidence of AMI, limit systolic BP drop to 10% of baseline or 25% if hypertensive

Dosages / Administration
A
- DO NOT SHAKE Metered-Dose Spray
- Pt should sit or lie down while administered
- Tablet and Metered-Dose Spray L/SL delivers 0.4mg
- ACS Dose: 0.4mg SL Tablet or L/SL Spray; May be given q 5min until CP free as long as BP remains >100 Systolic
- CHF Dose:
a. If pt is in mild distress and BP >100 Systolic; Give Nitro 0.4mg SL Tab or L/SL Spray; May repeat q 3-5min, if pt remains symptomatic, to Max Dose of 2mg
b. If pt in Moderate Distress, or severe Distress without AMS and BP>100 Systolic: Give Nitro 0.4mg; May repeat with 0.4mg SL Tab or 1-2Sprays q 3-5min, if pt remains symptomatic; Max Dose 2mg - IV Infusion for IFT set per releasing physician’s orders using IV pump.
