Amiodarone (Cordarone) Flashcards
1
Q
Class / Action
A
Antiarrhythmic - Rate control in a variety of atrial and ventricular tachyarrhytmia
2
Q
Onset / Duration
A
Immediate Onset / Up to 40days Duration of action
3
Q
Indications
A
- Shock refractory VF/pulseless VT
- Polymorphic VT/wide complex tachycardia of uncertain origin
- Control of hemodynamically unstable VT when cardioversion is unsuccessful
- Acceptable for termination of ectopic or multi-focal atrial tachycardia with preserved LV function
- Used for rate control in treatment of atrial fibrillation or flutter when other therapies are ineffective
4
Q
Contraindications
A
- Known Hypersensitivity
2. Pts with Cardiogenic Shock, marked sinus bradycardia, 2nd or 3rd Degree AV Block unless a pacemaker is available
5
Q
Precautions
A
- May produce vasodilation and hypotension
- May have negative inotropic effects and prolong QT interval
- Renal failure, terminal elimination is long (half-life lasts up to 40days)
6
Q
Dosages / Administration
A
- Cardiac Arrest
a. ADULT: First Dose: 300mg IV/IO Push. Second Dose if needed after 3-5min: 150mg IV/IO Push
b. PED: 5mg/kg IV/IO Push, up to max dose of 300mg; may repeat to Total Daily Dose of 15mg/kg - Wide Complex Tachycardia
a. Mix 150mg/100mL D5W / 10min IV/IO (15mg/min)
b. May repeat every 10min PRN
c. Consult Base Station if arrhythmia persists beyond second dose
7
Q
Adverse Reactions
A
Vasodilation, Hypotension, Bradycardia
