Heparin Mechanism?
Binds to antithrombin which blocks the conversion of prothrombin to thrombin and prevents the conversion of fibrinogen to fibrin thereby inhibiting factors 3 and 10.
Heparin Indications?
Treatment of thromboembolic disorders (DVT, PE)
Heparin Contraindications?
-Hypersensitivity
-Active Bleeding
-Recent intracranial, intraspinal, or eye surgery
-Severe hypertension
Heparin Side Effects/Precautions?
-Hemorrhage
-Hematuria
-GI Bleeding
-Shock
-Pain
-Bruising
-Thrombocytopenia
Heparin Adult Dose?
Bolus: 60 U/kg, max 4,000 U IV/SQ
Infusion: 12-15 U/kg/hr, max 1,000 U/hr
Heparin Pediatric Dose?
Not recommended
Heparin Onset of Action?
Immediate IV; 20-60 minutes SQ
Heparin Duration of Action?
4-8 Hours
Heparin Max Dose Adult/Pediatric?
1,000 - 4,000 Units depending on underlying condtion.
