Fibrinolytic-Thrombolytic Agent
Alteplase Mechanism?
Binds to fibrin-bound plasminogen at the clot site to convert plasminogen to plasmin. Plasmin digests the fibrin strands of theclot.
Alteplase Indications?
- Acute MI
- PE
- Ischemic stroke
Alteplase Contraindications?
- Active internal bleeding
- Surgery within 2-3 weeks
- Previous CVA
- Prolonged CPR
- Brain or spinal surgery within 3 months
- Brain cancer
- Aneurysm
- Recent significant trauma, especially head trauma
- Uncontrolled hypertension (>185/110)
Alteplase Side Effects/Precautions?
- Intracranial bleeding
- Headache
- Reperfusion dysrhythmias
- Chest pain
- GI bleeding
- Nausea/vomiting
- Abdominal pain
- Hypotension
Alteplase Adult Dose?
0.9 mg/kg; first 10% administered over one minute; remaining 90% administered over 60 minutes
Alteplase Pediatric Dose?
Not recommended
Alteplase Onset of Action?
Variable

Clot lysis with 60-90 minutes
