Sympathomimetic, Decongestants, Intranasal
Phenylephrine Mechanism?
Binds to A1 adrenergic receptors, second messenger release of calcum in VSMC, causing peripheral vasoconstriction, increasing SVR, increasing BP.
Phenylephrine Indications?
- Non-hypovolemic shock, especially distributive shocks
- Hypotension in the peri-intubation period when
using rapid sequence intubation
Phenylephrine Contraindications?
Hypovolemia
Tachydysrhythmia
Phenylephrine Side Effects/Precautions?
-Headache
-Syncope
-Tachycardia
-Hypertension
-Increased myocardial oxygen demand
-Dysrhythmia
-Nausea/Vomiting
Phenylephrine Adult Dose?
- Loading dose: 100-180 mcg/min IV/IO infusion until BP>90 systolic
- Maintenance dose: 40-60 mcg/min
- For RSI. 50-200 mcg slow IV push every 2-5 minutes as needed for hypotension
Phenylephrine Pediatric Dose?
0.1-0.5mcg/kg/min IV/IO infusion - titrate to normotensive systolic blood pressure

15-20 minutes
