Cholinergic, Toxicity Antidotes
Pralidoxime Mechanism?
Binds to organophosphates and breaks alkyl phosphate-cholinesterase bond to restore the activity of acetylcholinesterase. Primarily works at the nicotinic receptors
Pralidoxime Indications?
Suspected organophosphate poisoining
Pralidoxime Contraindications?
None in emergency setting
Pralidoxime Side Effects/Precautions?
Pain at injection site, blurred vision, diplopia, HTN, tachycardia, cardiac arrest, laryngospasm, muscle rigidity, headache, Sz, drowsiness, nausea
Pralidoxime Adult Dose?
600mg IM x 3 doses q 15 min; if symptoms persist, 1g IV/IO x 60 min
Pralidoxime Pediatric Dose?
< 40kg: 15mg/kg/dose IM x 3 dose q 15 min.
> 40kg: 600mg IM x 3 doses q 15 min
25 mg/kg IV/IO/IM (max 2 grams), rep. PRN within 30-60 min.
Pralidoxime Onset of Action?
5-15 minutes
Pralidoxime Duration of Action?
2 hours
Pralidoxime Max Dose Adult/Pediatric?
Adults and peds over 40kg: 1800mg. Peds <40kg: 45mg/kg
