Procainamide Classification?
Class I antidysrhythmic; sodium channel blocker
Procainamide Mechanism?
Binds to sodium channels which prolongs Phase 0 of the action potential thus decreasing ventricular automaticity. Little effect on atrial tissue. Suppresses intraventricular conduction
Procainamide Indications?
- Cardiac Arrest (VF, Pulseless VT)
- Stable Wide Complex Tachycardia
Procainamide Contraindications?
- Pre-existing Prolonged QT Interval
- High degree Heart block
- Hypersensitivity
- Hypotension
- Torsades de Pointes
- Digitalis toxicity
Procainamide Side Effects/Precautions?
- Dizziness
- Nausea
- Widening of PR, QRS, and QT intervals
- Bradycardia
- Heart block
- Hypotension
- Confusion
- Dysrhythmias
- Reflex Tachycardia
- Asystole
Procainamide Adult Dose?
20-50 mg/min slow IV/IO infusion until any one of following occur:
- Dysrhythmia is suppressed
- Hypotension
- QRS widens by > 50%
- Total dose of 17 mg/kg
Maintenance Infusion: 1-4 mg/min
Procainamide Pediatric Dose?
15 mg/kg slow IV/IO infusion until any one of following occur:
- Dysrhythmia is suppressed
- Hypotension
- QRS widens by > 50%
Procainamide Onset of Action?
10-30 Minutes
Procainamide Duration of Action?
3-6 Hours
Procainamide Max Dose Adult/Pediatric?
Adult: 50mg/kg/day Ped: 4g/day

IV/IO
