Adenosine
Adult: 6,12,12 IVP
Peds: 0.1mg/kg IVP, 0.2mg/kg max: 12mg
Albuterol
Adult & Peds: 2.5mg in 3mL nebulized. Max: None
Amiodarone
Adults: CA - 300mg, 150mg, 150mg PVT - 150mg in 100mL NS over 10 minutes. Max 2.2g
Peds: CA - 5mg/kg NTE 150mg, PVT 5mg/kg Max: 450mg
Atropine
Adults:
—-Brady - 0.5mg every 3-5, Max 3mg
—-Ophos - 2-5mg may repeat every 5 no max
Peds:
—–Brady - 0.02mg/kg every 3-5 May repeat once Max: 0.5mg
Calcium Chloride
Adults: 1g
Peds: 20mg/kg slow IVP over 5 min
Max: 2g for both
Calcium Gluconate
Adults: 2g
Peds: 60mg-100mg IVP over 5 min
Max: 2g for both
Dexamethasone
Adults: 0.6mg/kg
Peds: 0.6mg/kg
Max 10mg for both
Dextrose 10%
Adults: 25g IV infusion
Peds: 5mL/kg of D10
Epinephrine
Adults: CA 1mg (1:10,000) every 3-5 minutes, Shock/Brady 2-10mcg/min infusion, other 0.3mg 1:1,000
Peds: CA 0.01 mg/kg (1:10,000) every 3-5, Shock/Brady 0.1-1mcg/min infusion, other 0.1mg/kg 1:1,000
Etomidate
Adults: 0.3mg/kg

Glucagon
Adults: Hypo 1mg IM, others 3-5mg IVP
Peds: Hypo 0.01mg/kg, other 0.03-0.05mg/kg IVP
Hydrocortisone
Adults: 4mg/kg IVP

Ipratroprium Bromide
Adults: 0.5mg nebulized
Peds: 0.5mg nebulized

Lidocaine
Adults: CA 1.5mg/kg over 2-3 mintues, repeated by 0.75mg/kg every 5-10. MVT: 1.5mg/kg IV/IO
Peds: 1mg/kg IVP, 20-50mcg/kg/min
Max: 100mg for both
Magnesium Sulfate
Adults:
—Resp: 2g in 100mL over 10min
—Cardiac: 1g over 1min IVP
—Eclampsia: 4g in 100mL over 10min
Peds:
—Bronchospasm: 40 mg/kg in 100mL over 20 min.
—Pulseless TdP: 25-50 mg/kg (max 2 grams) IV/IO over 1-2 min.
