Class I antidysrhythmic; sodium channel blocker; local anesthetic
Lidocaine Mechanism?
Sodium channel blocker that prevents the rapid influx of sodium in the cardiac myocyte blunting depolarization during phase 0 of the action potential, thereby increasing the effective threshold potential
Lidocaine Indications?
Ventricular arrhythmias,
pVT,
monomorphic VT
Lidocaine Contraindications?
-Hypersensitivity
-Stokes-Adams Syndrome
-2d/3d degree AVB
-WPW
-CHF
-Cardiogenic shock
Lidocaine Side Effects/Precautions?
Hypotension, NV, confusion, dizziness, HA, cardiac arrest, dysrhythmia?s SZ
Lidocaine Adult Dose?
Cardiac arrest: 1.5mg/kg IV over 2-3 min, followed by 0.75 mg/kg q 5-10 minutes up to 3 mg/kg
Monomorphic VT: 1-1.5mg/kg IV/IO. Repeat x 1 after 5 min. up to 3mg/kg followed by 1-4 mg/min
Lidocaine Pediatric Dose?
Cardiac arrest: 1mg/kg up to 100mg followed by 20-50mcg/kg/min
Lidocaine Onset of Action?
45-90 seconds
Lidocaine Duration of Action?
10-20 minutes
Lidocaine Max Dose Adult/Pediatric?
3 mg/kg (100mg) for both

IV/IO
