A

- Periapical radiolucency persists
- Prolonged symtoms
- Retreatment impossible
- Perforation of root or pulp chamber which cannot be treated

- Local anatomical factors-inaccessible root
- Tooth w/ inadequate periodontal support
- Uncooperative patient
- Compromising medical condition
- Tooth unrestorable
