52. TMJ diseases: Arthritis of the mandibular joint ± Flashcards
1
Q
TMJ diseases and types of arthritis affecting the mandibular joint
A
- Disorders affecting the temporomandibular joint (TMJ)
- Types =>infectious, traumatic, rheumatoid, and secondary degenerative arthritis
2
Q
Main causes (etiology) of TMJ arthritis
A
- Trauma
- Infection
- Cartilage degeneration=>Natural wear and tear.
- Systemic polyarthritic or collagen diseases= rheumatoid arthritis
3
Q
Common clinical features (clinics) of inflammatory joint conditions affecting the TMJ
A
- Localized joint pain
- Limited jaw movements
- Swelling and tenderness
- Possible systemic symptoms =>depending on the underlying cause
4
Q
Infectious arthritis of the TMJ
A
- Infection of the TMJ=> adjacent infection or via bloodborne spread=>
- Localized joint pain
- Limited jaw movements
- Swelling and tenderness
- Possible systemic symptoms
- Antibiotics, hydration, pain control, and movement restriction
Antibiotics-(initially parenteral penicillin G, switching based on culture results),
5
Q
A
6
Q
Traumatic arthritis of the TMJ
A
- Acute injuries => difficult tooth extractions or endotracheal intubation=>
- Localized joint pain
- Limited jaw movements
- Swelling and tenderness
- NSAIDs, heat application, a soft diet, and jaw movement restriction
7
Q
Rheumatoid arthritis (RA) of the TMJ
A
- Affects the TMJ in over 17% of adults and children with RA
- Localized joint pain
- Limited jaw movements
- Swelling and tenderness
- Tx=> NSAIDs, restricted jaw function
- Night-time mouth guard or splint
-In children, it can cause mandibular growth disturbances and facial deformity.
8
Q
Secondary degenerative arthritis of the TMJ
A
- People aged 20 to 40 years following trauma or w/ persistent myofascial pain syndrome
- Localized joint pain
- Limited jaw movements
- Swelling and tenderness
- Crepitus
- Tx=>Conservative=> occlusal splint (worn constantly except during meals, oral hygiene, and cleaning
