22. Odontogenic cysts of the jaws. Classification, etiology, pathogenesis, clinical features, diagnosis, differential diagnosis and treatment. Complications Flashcards
1
Q
Types of odontogenic cysts
A
- Radicular
- Eruption
- Residual
- Dentigerous
- Odontogenic keratocyst
- Gingival cyst
- Lateral periodontal cyst
2
Q
Radicular cyst
A
- Most common jaw cyst
- Develop from periapical granuloma from remnants of pulp→chronic inflammation stimulates cell rests of malassez→Epithelial proliferation→phase of development and growth
- Fluid filled(expand in jaw by osmotic pressure
- Cytokines cause local resorption of bone
3
Q
Clinical presentation of radicular cyst
A
- Firm swelling at apex of tooth
- Bone resorption leaving soft swelling that gives way on palpation(shell crackling)
- Pain on palpation
4
Q
Investigations of Radicular cysts
A
- Vitality testing
- Periapical and panoramic→smooth rounded, unilocular radiolucency w/ radiopaque outline(at apex of non vital tooth)
- Adjacent structures may be displaced by cyst
5
Q
Treatment methods for Radicular cysts
A
- Root filling
- Extraction
- Enucleation for large cysts
- Curetting cyst lining
- Apicectomy
6
Q
Residual cyst
A
- From residual periapical infection
- Cyst fragments from extraction of non vital tooth
- Similar features to radicular cysts
7
Q
Residual cyst radiographically
A
-Isolated radiolucent lesions in alveolar process(w/out obvious cause)
8
Q
Residual cyst treatment
A
Enucleation or Marsupialisation
9
Q
Lateral periodontal cysts
A
- Arise from cell rests within PDL
- Adjacent teeth vital
- Asymptomatic
10
Q
Lateral periodontal cysts radiographically
A
- Between roots of teeth
- Mandibular canine and premolar area
11
Q
Lateral periodontal cysts treatment
A
- Enucleation
- Extraction somtimes
12
Q
Dentigerous cysts
A
- Asymptomatic swelling
- Expansion of dental follicle-may affect eruption and displace teeth
- Attached to crown at CEJ
Mandibular third molar and maxillary canine
13
Q
Dentigerous cyst radiographically
A
- Circumscribed, unilocular, radiolucent area=>
- w/ thin radiopaque lamina dura associated w/ crown
- Cyst in maxilla may occupy crown
14
Q
Dentigerous cyst treatment
A
- Definitive surgery
- Enucleation
- Treatment of cavity - carnoys solution(kills residual cells in bony spaces)
- Removal of overlying mucosa(removal of overlying dental lamina rests)
15
Q
Calcifying epithelial odontogenic cyst
A
- Rare
- Interosseous or in gingiva
- Benign but can become aggressive and recur
