Q

What is the idea of foetal programming?

A

The environment in the womb can alter the development of the foetus with a long-lasting effect on the child

Q

What is sensitive early mothering useful for?

A

It helps attachment and can counteract some negative programming that occurred in the womb

Impaired cognitive development
Neonatal behaviour
More difficult infant temperament
Victimisation in childhood (bullying)
Schizophrenia (stress in first trimester, to do with neuron migration)
Autism spectrum
Personality disorder

Q

Why are there associations between prenatal stress and increased risks for children of many conditions?

A

A lot of it is due to genetics but also due to environmental factors

Q

What is COMT?

A

Gene variants associated with working memory and ADHD, they inactivate catecholamines

9
Q

What have studies looking at COMT shown about prenatal stress and effects on the child?

A

Certain genes will mean a child is affected by maternal prenatal stress whereas certain genes will stop them from being affected so how they are effected is dependant on their own genetic composition

12
Q

What enzyme is suspected to cause the effects on the baby as a result of maternal stress/mental illness? How does it enter the baby?

A

Cortisol (and maybe pro inflammatory cytokines), affects the baby via transplacental passage

13
Q

How does maternal stress/mental illness result in the foetus being exposed to more cortisol?

A

During periods of stress or in mental illness, the mother produces more cortisol
The more ill/stressed the mother is, the less they can break down cortisol in the placenta so the fetus is more exposed to it

15
Q

What factor affects how much effect cortisol has on the baby?

A

Maternal attachment, if attachment is insecure cortisol levels have a greater effect on IQ

16
Q

What are epigenetic changes?

A

Functionally relevant modifications to the genome that do not involve a change in nucleotide sequence. Can persist to grandchild generation

17
Q

As a health professional what can you do to help the mother and fetus’ health?

A

Detect and treat anxiety and depression both in pregnancy and postnatally by using psychological interventions (eg CBT0 or pharmacological intervention if needed
Help with relationship problems or domestic abuse
Help to create more social support
Practical help with housing etc

18
Q

What are conservative ways antenatal stress can be reduced?

A

Yoga
Gardening
Music
Stroking the belly

19
Q

Why is breastfeeding beneficial?

A

Breastfed babies have a higher IQ and lower levels of ADHD (but its the content of the milk that matters, breast milk can be given by bottle and will still have the benefits)
