What is developmental dysplasia of the hip?
Abnormal developmental of the fetal hip bones causes a structural abnormality
What are children with developmental dysplasia of the hip susceptible to?
Dislocation and subluxation of the hip
What moves are performed during the newborn check to screen for developmental dysplasia of the hip?
Ortolani and Barlow manoeuvres
What are the risk factors for developmental dysplasia of the hip?
Female
Firstborn
Family history
Breech
Fluid (too little amniotic fluid)
What investigation should be ordered if developmental dysplasia of the hip is suspected?
Hip ultrasound
How is the Barlow manoeuvre performed?
The hip and knee are flexed, and the hip is pushed backwards to see if it dislocated posteriorly
How is the Ortolani manoeuvre performed?
Gentle pressure is used to abduct the hips (with the hips and knees flexed) to see if the hip will dislocate anteriorly
What is the first line management of developmental dysplasia of the hip?
A Pavlik harness to keep the hip abudcted and flexed
What is the definitive investigation for diagnosis of DDH?
Hip ultrasound
When is surgery needed for DDH?
When a pavlik harness fails, or a diagnosis is made after 6 months of age
What are the complications of developmental dysplasia of the hip?
Premature joint degeneration
Arthritis
Chronic lower back pain
Avascular necrosis
