Describe the dosing of tacrolimus
The target concentration depends on several factors including the indication, use of other immunosuppressants, time since transplant, whether rejection or adverse effects have occurred, and the assay method. Trough whole blood concentrations are usually used for oral dosing; refer to the relevant specialist unit for specific recommendations. After a dose, brand or formulation change, it may take 3–5 days to reach a stable trough concentration.
