What is a practical limitation of azATHIOPRINE in IBD?
It takes months to work
How is azATHIOPRINE normally initiated in hospital for IBD patients?
Normally, patients will start 40 mg prednisolone when they start azATHIOPRINE and taper the prednisolone at 5 mg/week, so by the time the prednisolone course is completed, azATHIOPRINE should have taken effect.
What is the risk of interaction between xanthine oxidase inhibitors and azATHIOPRINE?
Xanthine oxidase inhibitors increase the risk of bone marrow suppression with azATHIOPRINE.
Describe the deliberate use of interaction between xanthine oxidase inhibitors and azATHIOPRINE.
Very rarely this is done deliberately to boost responses in patients with high TPMT activity. This is extremely rare and as such you should query it with the team every single time you see it.
