Expected Pharmacologic Action
A
thin and enhance the flow of secretions in the respiratory passages

Therapeutic Use
A
- acute and chronic pulmonary disorders exacerbated by excessive/ large amount of secretions
- used in cystic fibrosis
- acetylcysteine is an antidote for acetaminophen poisoning

Complications
A
- when administered orally: aspiration and bronchospasm
- dizziness, drowsiness, hypotension, tachycardia (CNS)
- hepatotoxicity

Medication Administration
A
- by inhalation to liquify nasal and bronchial secretions and facilitate coughing
- orally or IV for acetaminophen toxicity
- be prepared to suction if aspiration occurs with oral administration

Contraindications
A
- pregnancy risk category B
- not used in clients who are hypersensitive to acetylcysteine
- use cautiously in hypothyroidism, CNS depression, renal or liver disease, and seizure disorders
- caution in patient with asthma due to potential for bronchospasms

Nursing Interventions
A
- monitor vital signs
- monitor liver function tests
- monitor PT, BUN, Creatinine, Glucose, Electrolytes and acetaminophen levels in clients who have acetaminophen toxicity

A
-improvement of manifestations as demonstrated by regular respiratory rate, clear lung sounds, and increased ease of expectoration
