Action

A
  • Beta1 (cardioselective) and beta2 (non-selective): decrease HR, decreased myocardial contraction (decrease CO), decrease rate of AV conduction
  • alpha and beta blockers: alpha adds vasodilation

Complications

A
  • Beta 1 blockade: bradycardia, decreased CO, AV block, orthostatic hypotension, rebound myocardium excitation
  • beta 2 blockade: bronchoconstriction, glycogenolysis (breakdown of glycogen) is inhibited

Nursing considerations

A
  • Beta1 blockade: monitor VS, hold HR <50, use with caution for HF, monitor for signs of worsening HF, baseline ECG, myocardium becomes sensitized to catecholamines with long term use, do not stop abruptly, d/c over 1-2 weeks
  • beta2: avoid in asthmatics, diabetic mellitus patients rely on the breakdown of glycogen into glucose to manage low blood sugars, monitor blood glucose levels
