What is papilledema
A
Swelling of the optic disc
*increased intracranial pressure
*painless, passive, typically bilateral

What can papilledema lead to?
A
Early on it may have vision issues
*enlargement of blind spot without losing acuity

If you see papilledema during a PE what must you do?
A
Immediate search for cause

What are the signs of papilledema
A
- Headache
- Nausea, vomiting
- Binocular horizontal diplopia
- Pulsatile machine-like sound in ear
- Flame-shaped hemorrhages in nerve fiber layer adjacent to the disc
- Enlarged blind spot on visual field

How to diagnose papilledema
A
Emergent diagnosis
*MRI with gandolinium contrast preferred CT
*lumbar puncture possible

What is the treatment of papilledema
A
Treat the underlying cause
1. Initially diuretics (decreased IOP)

What is the prognosis of papilledema
A
- Chronic papilledema can lead to progressive vision loss (peripheral fields) and eventually complete vision loss
