What causes a hordeolum
Staphylococcal abscess
What increases the incidence of a hordeolum
- History of blepharitis
- Rosacea
- Seborrheic dermatitis
- Poor hygiene
- Hormonal changes
What are the signs of a hordeolum
Localized, red, swollen, acutely tender area
1. Discharge, tearing, mild blurry vision, burning, or scratchy sensation
2. Upper or lower eyelid
How is a hordeolum diagnosed
Clinically
What is an internal hordeolum
- Meibomian gland abscess that points towards the conjunctival surface of the lid
- Evert eyelid to visualize
What is an external hordeolum
AKA “sty”
1. Glands in eyelash follicles or on lid margin
*glands of Zeis or Moll
What is the treatment of a hordeolum
Supportive care, warm compresses
1. Meticulous eyelid hygiene
Does ABx ointment help with the healing of a hordeolum?
No
When to consider to send a patient to the ophthalmologist
If signs worsen, persists beyond expected time period
*I and D considered
What are the complication of a hordeolum
Internal hordeolum can lead to chalzion and generalized, peri-orbital cellulitis of the lid
What are some aspects of meticulous eyelid hygiene
- Wash hands
- No touching
- Do not wear contact lenses
- Do not share eye meds, cosmetics, contact lenses, discard and change
- Do not press or squeeze
- Clean eyelids with wet/warm cotton balls, Q-tips, baby shampoo
