Mechanism of Action
A
Oxytocin binds to oxytocin receptors in uterine SMC, initiates activity of second messengers, which results in increased intracellular calcium, and in turn promotes contraction of uterine muscle

Pharmacokinetics
A
- Onset of Action- immediate IV, 3-7min IM
- Peak Effects- varies
- Duration of Effects- 1 hour IV, 2-3 hours IM
- Half-Life- 3-5 minutes

Post-partum Hemmorhage

Contraindications
A
- Baby and placenta not delivered
- Additional fetus present
If given prior to fetal delivery, fetal hypoxia, asphyxia, intracerebral hemorrhage may occur

Side Effects/Adverse Reactions
A
Hypotension, tachycardia, dysrhythmias, seizures, coma, N&V

Dose
A
Adult:
- 3-30 units IM - 10-20 units in 500mL or 1000mL D5W, NS or LR- slow IV/IO infusion

Precautions
A
Can cause over-stimulation of uterus and possible uterine rupture- monitor uterine tone
