Mechanism of Action
A
- Primary Class III antiarrhythmic/K+ channel blocker-> prolongs phase 3 repolarization and refractory period of ventricular action potential
- Secondary Class 1A antiarrhythmic/Na+ channel blocker-> decreases amplitude of phase 0 depolarization of ventricular action potential
- Secondary Class II antiarrthyhmic/Non competitive Beta blocker
- > Negative chronotropy, inotropy, dromotropy
- Secondary Class IV antiarrhythmic/weak Ca2+ channel blocker
- > negative chronotropy, inotropy, dromotropy

Cardiac Arrest (VF, pulseless VT) Wide Complex Tachycardia

A
- Bradycardia
- 2/3 degree heart block
- Hypersensitivity

Hypotension, bradycardia, dyspnea, cough, dizziness, prolonged PR, QRS, QT intervals

Dose
A
Adult:
- VF/Pulseless VT- 300mg IV/IO, repeat dose 150mg IV/IO in 3-5min PRN - VT with pulse- 150 mg IV/IO over 10 min - Maintainance- 1mg/min first 6 hours, then 0.5mg/min
Pedi:
- VF/Pulseless VT- 5mg/kg IV/IO; max 300mg, max total dose 15mg/kg - VT with pulse- 5mg/kg IV/IO, over 20-60min; max 300mg

Precautions
A
Caution in patients with heart failure

Drug-Drug Interactions
A
- Reduces renal and non-renal clearance of digoxin
- Decreased bio transformation and clearance of warfarin -> enhanced anti-coagulation
- Potentiates procainamide and quinidine
