Narcotic Analgesic
Mechanism of Action
Opiate R agonist -> Hyperpolarization of interneurons and decreased release of neurotransmitters responsible for the transmission of pain impulses to the thalamus and cerebral cortex
Pharmacokinetics
- Onset of Action- Immediate
- Peak Effects- 3-5 minutes IV
- Duration of Effects- 30-60 minutes
- Half-Life- 6-8 hours
Indications/Field Use
- Severe Pain
- Adjunct for RSI
Contraindications
- Respiratory depression/insufficiency when considering for pain management
- Uncorrected hypotension
- Known hypersensitivity
Side Effects/Adverse Reactions
Apnea/respiratory depression, bradycardia, chest wall rigidity
Dose
Adult:
1mcg/kg slow IV/IO/IN max single dose 100 mcg, repeat after 10 minutes
>65- 0.5mcg/kg slow IV/IO/IN max single dose 50 mcg, repeat in 5 minutes
Pedi:
1-2mcg/kg slow IV/IO/IN
Precautions
Caution in patients with kidney and liver dysfunction
Drug-Drug Interactions
- Additive effects with alcohol
- Shouldn’t be given with patients using MAOI in the last 14 days- may cause serotonin syndrome
