What is ovarian torsion?
Partial or complete torsion of the ovary on it’s supporting ligaments that may in turn compromise the blood supply
What can ovarian torsion compromise?
The blood supply to the ovary
What are the main risk factors for ovarian torsion?
ovarian mass
being of a reproductive age
pregnancy
ovarian hyperstimulation syndrome
What are the main symptoms of ovarian torsion?
Sudden onset of deep-seated colicky abdominal pain.
Vomiting and distress
Fever
What might you find on a vaginal examintion of someone with an ovarian torsion?
Adnexial tenderness
What imaging might you use in someone with a suspected ovarian torsion?
Ultrasound
What might you see on an ultrasound of someone with ovarian torsion?
Whirlpool sign
What is both diagnostic and theraputic for someone with ovarian torsion?
Laparoscopy
What is an ovarian torsion usually due to?
Ovarian mass larger than 5cm- cyst or a tumour
What does twisting of the adnexa and blood supply cause?
Twisting of the adnexa and blood supply to the ovary leads to ischaemia. If the torsion persists, necrosis
What type of situation is ovarian torsion?
Surgical emergency
What are the main signs of ovarian torsion?
Sudden onset severe unilateral pelvic pain
Nausea and vomiting
What does whirlpool sign describe?
Free fluid in pelvis and oedema of the ovary
What can be done with laparoscopic surgery to treat an ovarian torsion?
Un-twist the ovary and fix it in place (detorsion)
Or
Remove the affected ovary (oophorectomy)
