What is endometriosis?
A condition where ectopic endometrial tissue grows outside the uterine cavity
What are clinical features of endometriosis?
chronic pelvic pain
Dysmenorrhoea (painful period)
Dyspareunia (pain during sex)
Subfertility
When does the pain normally occur in endometriosis?
Dyas before the bleeding
What non-gynaecological symptoms can endometriosis present with?
Dysuria
Urgency, Haematuria
Dyschezia-Painful bowel movements
What is the gold standard investigation for endometriosis?
Laparoscopy
Why isn’t a laparoscopy first line?
There is a small risk of bowel perforation
What would a transvaginal ultrasound show?
Nothing
It is usually normal
What might you find on a pelvic examination?
Reduced organ mobility
Tender nodularity in the posterior vaginal fornix
Visible vaginal endometriotic lesions may be seen
What is the management of endometriosis dependent on?
The symptoms and the severity
What would you give for mild pain in endometriosis?
NSAID’s
Paracetamol
What would you give if the analgesia didn’t help the endometriosis?
Hormonal treatment
What hormonal treatment could you give for endometriosis?
Combined oral contraceptive pill
Progesterones
What progesterone’s could you give for endometriosis?
Medroxyprogesterone acetate
What would be more suitable for managing infertility than menstrual suppression?
Laparoscopic excision or Ablation surgery
What medical management would you give if fertility was the priority
GnRH analogues
