What is cord prolapse?
The umbilical cord exits the cervix prior to delivery of the baby
When does cord prolapse occur?
During labour
What can happen if cord prolapse is left untreated?
It can lead to compression of the cord or cord spasm
What can cord compression or cord spasm cause?
Fetal hypoxia
Irreversible damage and death
How is cord prolapse diagnosed?
Abnormal fetal heart rate
Cord is palpable vaginally
The cord is visible beyond the level of the introitus
What are the main risk factors of cord prolapse?
Multiparity
Polyhydramnios
High fetal station
Long umbilical cord
What is the main aim of the management of cord prolapse?
To delivery the baby as fast as possible
What are the 2 ways that the baby can be delivered?
Caesarean section
Instrumental vaginal delivery (if cervix is fully dilated and head is low)
What additional medical management can you give for cord prolapse?
Tocolytics e.g. terbutaline
What do tocolytics do?
Stop uterine contractions
What can you do as an aid in preventing cord prolapse?
Fill the bladder with 500ml warmed saline
What position should the mother adopt?
All fours position
What is the main cause of cord prolapse?
Artificial amniotomy: Artificial rupture of membranes
What happens due to cord compression?
Placental blood flow to the foetus becomes compromise
Foetal bradycardia and decelerations.
What is the first step when cord prolapse is noted?
Push the presenting part of the fetus back in
