Opiate Analgesic
Morphine Mechanism
Opiate R agonist -> hyperpolarization of interneurons and decreased release of NT responsible for the transmission of pain impulses to the thalamus and cerebral cortex
Morphine Pharmacokinetics
Onset: Immediate IV, 15-30m IM
Peak Effects: 20m IV, 30-60m IM
Duration: 2-7h
Half Life: 1-7h
Morphine Indications
Severe pain
Morphine Contraindications
- Hypotension
- Significant head trauma with AMS
Morphine Side Effects/Adverse Reactions
N/V, Abd cramps, AMS, HA, blurred vision, constricted pupils
Morphine Dose
Adult:
0.1mg/kg slow (2mg/min) IV IO IM (max single dose = 10mg) q10m up to 3 total doses
>65y/o: 0.05mg/kg slow (2mg/min) IV IO IM (max single dose = 10mg) q10m up to 3 total doses
Pedi:
0.1mg/kg slow IV IO IM
Morphine Precautions
- Causes decreased chemoreceptor responsiveness to increased levels of CO2 -> may lead to respiratory depression
- Caution in pts with hypersensitivity to analogs of morphine
Morphine Drug Interactions
Additive effects (CNS depression) when given with antihistamines, antiemetics, sedatives, hypnotics, barbiturates, and EtOH
