MIOSHA: Summary: Part 12 Scaffolds and Scaffold Platforms Flashcards
Cleat means X.
a structural block used at the end of a platform to prevent the platform from slipping off its supports.
Cleats are also used to
provide footing on sloped surfaces such as crawling boards
Competent person means
a person who is experienced and capable of identifying an existing or potential hazard in surroundings, or under working conditions, that are hazardous or dangerous to an employee and who has the authority and knowledge to take prompt corrective measures to eliminate the hazards.
Crawling board sometimes called a chicken ladder , means
a plank that has cleats, which are spaced and secured, at equal intervals for use by an employee on roofs.
Form Scaffold means
a supported scaffold that consists of a platform supported by brackets attached to the form work .
A crawling board is not designed to
carry any material.
Heavy-duty scaffold means
a scaffold that is designed and constructed to carry a working load of not more than 75 pounds per square foot.
Horse Scaffold means
a supported scaffold that consists of a platform supported by construction horses (sawhorses). Horse scaffolds constructed of metal are sometimes known as trestle scaffolds.
Ladder jack scaffold means
a scaffold that is supported by brackets attached to ladders.
Lean-to scaffold means
a supported scaffold that is kept erect by tilting it toward, and resting it against, a building or structure.
Mobile scaffold means
a powered or unpowered portable caster or wheel-mounted supported scaffold.
Pump-jack scaffold means
a scaffold for light-duty work that consists of vertical poles, platform planking, and movable brackets for raising or lowering the platform on the vertical poles by a manual pumping action.
Qualified person
means one who, by possession of a recognized degree, certificate, or professional standing, or who by extensive knowledge, training, and experience, has successfully demonstrated
his or her ability to solve or resolve problems related to the subject matter, the work, or the project.
Roof bracket scaffold means
a rooftop-supported scaffold that consists of a platform resting on angular-shaped supports.
Rough terrain forklift truck means
a wheeled-type truck which is designed primarily as a fork truck that has a vertical mast or pivoted boom, or both, and that is intended for operation on unimproved natural terrain as well as the disturbed terrain of construction sites.
Tubular welded frame scaffold or “fabricated frame scaffold” means
a scaffold platform that is supported by a metal sectional frame that consists of posts and a horizontal bearer that has intermediate members.
B. Training requirements
An employer shall have each employee who performs work on a scaffold trained by a person qualified in scaffold safety. The training shall enable an employee to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize the hazards. The training shall include the following areas as applicable:
a.
b.
c.
a. Electrical hazards.
b. Fall protection.
c. Material loads
B. Training requirements
An employer shall have each employee who is involved in erecting, disassembling, moving, operating, repairing, maintaining, or inspecting a scaffold X
trained by a competent person to recognize any hazards associated with the work in question.
C. Construction and capacity generally.
A scaffold shall be designed, constructed, erected, and used in accordance with the provisions of this part. A scaffold shall be designed by X
a qualified person.
C. Construction and capacity generally.
A scaffold shall not be X, except under the supervision of a competent person.
erected, moved, dismantled, or altered
C. Construction and capacity generally.
A scaffold and its components shall be capable of supporting, without failure, not less than X times the maximum intended load.
4
C. Construction and capacity generally.
An employee on a scaffold who is exposed to an overhead hazard of falling material shall be protected with X
overhead protection that is sufficient to prevent injury.
C. Construction and capacity generally.
All load-carrying wood members of scaffold framing shall be a minimum of X psi fiber stress value.
1,500
C. Construction and capacity generally.
A X or Y shall not be used.

lean-to scaffold
C. Construction and capacity generally.
An employee shall not be allowed within X feet of uninsulated energized electrical lines.
10
D. Guardrails; fall arrest devices
A guardrail shall be installed on any open side or end of a scaffold work platform that is X or more feet above the floor or ground.
10
D. Guardrails; fall arrest devices
An employer shall have a competent person determine the feasibility and safety of providing fall protection for X.
An employer is required to provide
fall protection for employees erecting or dismantling supported scaffolds where Y
employees erecting or dismantling supported scaffolds
the installation and use of the protection is feasible and does not create a greater hazard.
E. Planking and scaffold platforms generally
If wood planks are used for a work platform, then the planks shall be scaffold-grade lumber that has a minimum of X pounds per square inch fiber stress value.
The planks shall be not less than Y inches by Z inches. The platform shall consist of a minimum of Y
planks laid side by side. Each platform on all working levels of scaffolds shall be fully planked or decked between uprights where practicable. Spaces between the platform and the uprights shall not
be more than W inches.

E. Planking and scaffold platforms generally
Where planks are lapped, each plank shall lap its bearer not less than X inches, which will provide a minimum overlap of Y inches.

12
E. Planking and scaffold platforms generally
Where a scaffold turns a corner, the planks shall be laid to X. The planks that run in the different direction shall be laid so as to Y
prevent tipping
extend over the rest on the first layer of planks.
E. Planking and scaffold platforms generally
When moving a platform to the next level, an employee shall leave X until the new platform supports have been set in place.
the old platform undisturbed
E. Planking and scaffold platforms generally
A platform shall not deflect more than X of the span when loaded.
1/60
E. Planking and scaffold platforms generally
The front of a platform shall be not more than X inches from the face of the work unless a guardrail system is erected along the front edge, or unless a personal fall arrest system is used, the maximum distance from the face of the work for plastering and lathing operations shall be not more than Y inches.

F. Protection from falling objects
In addition to wearing a hard hat, an employee on a scaffold shall be provided with additional protection from falling hand tools, debris, and other small objects through the installation of X or through the erection of Y that contain or deflect the falling objects.
toeboards, screens, or guardrail systems
debris nets, catch platforms, or canopy structures
F. Protection from falling objects
If there is a danger of tools, materials, or equipment falling from a scaffold and striking employees below, then one of the following provisions shall apply:
The area below the scaffold to which objects can fall shall be X
barricaded and employees shall not be permitted to enter the hazard area.
F. Protection from falling objects
If there is a danger of tools, materials, or equipment falling from a scaffold and striking employees below, then one of the following provisions shall apply:
If tools, materials, or equipment are piled to a height higher than the top edge of the toeboard, then X shall be erected for a distance sufficient to protect employees below.
paneling or screening extending from the toeboard or platform to the top of the guardrail
