  • Deep digital sepsis/septic arthritis of DIP joint
  • Retroarticular abscess
  • Non-healing wall lesions
  • Toe necrosis

A

Method 1- disarticulation of PIP
Remaining cartilage causing problems

How is the PIP disarticulated for amputation?

A
  • Foot washed and disinfected
  • IV regional block- tourniquet, 20-30ml local, 19-21G butterfly catheter
  • Cut through
  • Bandaged
  • Antibiotics usually a good idea

How is arthrodesis performed?

A
  • Indicated with deep infection of the joint- destroy the joint and effuse it
  • Foot washed and disinfected
  • IV regional block- tourniquet, 20-30ml local, 19-21G butterfly catheter
  • Open joint capsule, remove navicular and visualise joint
  • Drill holes add draining tubes
  • Remove 2/3 days later

A
  • Dressing changes
  • ABs
  • Analgesia

A
  • Poor post op care
  • Problems with remaining claw (amputation)
  • Ascending infections
  • Chronic pain
  • Failure to achieve ankylosis of the joint
