What are the indications for digital amputation?
A
- Deep digital sepsis/septic arthritis of DIP joint
- Retroarticular abscess
- Non-healing wall lesions
- Toe necrosis

What method is prefered?
A
Method 1- disarticulation of PIP
Remaining cartilage causing problems

How is the PIP disarticulated for amputation?
A
- Foot washed and disinfected
- IV regional block- tourniquet, 20-30ml local, 19-21G butterfly catheter
- Cut through
- Bandaged
- Antibiotics usually a good idea

How is arthrodesis performed?
A
- Indicated with deep infection of the joint- destroy the joint and effuse it
- Foot washed and disinfected
- IV regional block- tourniquet, 20-30ml local, 19-21G butterfly catheter
- Open joint capsule, remove navicular and visualise joint
- Drill holes add draining tubes
- Remove 2/3 days later

What post operative care is needed?
A
- Dressing changes
- ABs
- Analgesia

What can go wrong post operatively?
A
- Poor post op care
- Problems with remaining claw (amputation)
- Ascending infections
- Chronic pain
- Failure to achieve ankylosis of the joint
