Exam 2 week 8 ppt 7 Somatosensory Clinical Correlations Flashcards
Define Neuropathy:
•disease of the peripheral nerves
three types of neuropathy
- Mononeuropathy
- Radiculopathy
- Polyneuropathy
mononeuropathy
§Affecting one nerve such as carpal tunnel syndrome, damage to radial nerve with humeral fracture or common fibular nerve with fibular head fracture
radiculopathy
§Affecting nerve roots with spinal nerve distribution
polyneuropathy
§Affecting many nerves such as diabetes
what is one of the most common causes of of polyneuropathy
Diabetes mellitus
describe polyneuropathy from diabetes Mellitus:
–Characterized by distal, bilateral sensory losses
–Muscle wasting and weakness
–Progresses from distal to proximal – often called stocking-glove pattern
Brown Sequard Syndrome
•Lesion of one-half of the spinal cord and all fibers entering the cord via the dorsal root producing:
- –Ipsilateral zone of somatosensory anesthesia at the damaged dermatomes
- –Ipsilateral loss of DC-ML modalities below the lesion
- –Contralateral loss of STT modalities below the lesion
- –Ipsilateral spastic paralysis
Syringomyelia
- •Disease process in the center of the spinal cord, usually over a few cervical segments
- •Disrupts pain and temperature fibers of the STT in the ventral white commissure
- •Characterized by bilateral loss or pain and temperature sensibility, usually in the hands and arms
Syringomyelia is a Disease process in the center of the spinal cord, usually over a few cervical segments
Disrupts pain and temperature fibers of the STT in the ventral white commissure and so is Characterized by bilateral loss or pain and temperature sensibility, usually in the hands and arms
Subacute Combined Degeneration
- •Disorder that typically first involves the fibers of the dorsal columns
- •Early symptoms include symmetrical parathesias of fingers and toes and loss of proprioception
- •Person may demonstrate unsteady, ataxic gait
Subacute Combined Degeneration is a Disorder that typically first involves the fibers of the dorsal columns
Early symptoms include symmetrical parathesias of fingers and toes and loss of proprioception. As the disease progresses the patient may demonstrate unsteady, ataxic gait
Examination of the Somatosesory Systems: Three things about them
- •Relies on capacity of patient to cooperate and report feelings and sensations
- •Clinical tests are only remotely related to real-life sensory experiences
- •Tests for each spinal cord tract
- –Anterolateral-spinothalamic tract
- –Dorsal column–medial lemniscus
Evaluation of the somatosensory system Relies on capacity of patient to cooperate and report feelings and sensations. Clinical tests are only remotely related to real-life sensory experiences due to the testing of isolated sensory functions. There are Tests for each spinal cord tract with separate tests for the Anterolateral-spinothalamic tract and Dorsal column–medial lemniscus
do we need to know the Tests to Assess Somatosensory System Integrity (Tabel 9-4)?
No not in this class right now according to Dr. Lake
