What is diabetes insipidus key symptoms?
Excess urine
Extreme thirst
Two types of diabetes insipidus?
- Pituitary (Cranial)
- Nephrogenic (Partial)
What is pituitary diabetes insipidus?
Lack of vasopressin (ADH) production
Treatment for pituitary diabetes insipidus?
Vasopressin or Desmopressin
What is Nephrogenic (Partial) diabetes insipidus?
Kidneys that do not respond to to ADH
Nephrogenic diabetes insipidus treatment?
Thiazides - paradoxical effect (opposite)
Why Desmopressin instead of vasopressin?
Desmopressin more potent and last longer than vasopressin.
No vasoconstrictor effect
Side effects of Desmopressin?
- Hyponatraemia (CDDS)
- Nausea
If there is too much ADH what happens?
Too much water is stored, salt dilutes therefore Hyponatramia
Treatment of Too much ADH?
1) Fluid restriction
2) Democlocycline
3) Tolvaptan (Vasopressin antagonist)
Democlocycline work in too much Adh secretion?
Blocks renal tubular effect of ADH,
Allowing more water to be excreted
Why avoid rapid correction of Hyponatraemia with Tolvaptan?
Can lead to osmotic demyelination ( serious neurological event)
