What are the three types of insomnias?
- Transient Insomnia
- Short term insomnia
- Chronic insomnia
What is transient insomnia?
External factor caused insomnia by for example: jet lag, shift work, noise etc
Transient insomnia treatment?
Use Rapidly eliminated hypnotics,
1 or 2 doses only
Start with OTC like Nytol
What is Short term insomnia?
Caused by emotional problem or serious medical illness
Short term insomnia treatment
Hypnotic maximum 3 weeks use as can become dependent
(Preferably 1 week)
What is chronic insomnia?
Secondary insomnia
Caused by factors such as:
Alcohol
Anxiety
Depression etc
Chronic insomnia treatment?
Treat the UNDERLYING PSYCHIATRIC COMPLAINT rather than predominantly the insomnia which has been caused as a result of
Long acting benzodiazepines?
Diazepam
Nitrazepam
Flurazepam
DNF
Short acting Benzodiazepines?
Loprazolam
Lormetazepam
Temazepam
LLT
Hangover effect in long acting benzodiazepines?
Higher effect the following day
Hangover effect in short acting benzodiazepines?
Little or No hangover effect
Which is used for sleep maintenance?
Long acting Benzodiazepines
(Sedate through the night)
Which is used for sleep onset?
Short acting benzodiazepines
Such as:
Loprazolam, lormetazepam and Temazepam
Z - Hypnotics? Max day use? Max week use?
Zolpidem and Zopiclone
Max 3-4 days use (intermittently)
Max 4 weeks use
Which drugs cause a risk of fall in elderly therefore should be avoided?
Benzos and Z hypnotics (Zolpidem, zopiclone)
