1) SABA or SAMA
Non Asthmatic COPD treatment
1) SABA or SAMA
2) Then LAMA + LABA (Discont. SAMA)
3) LAMA + LABA + ICS
Step 3 only occurs if:
- pt is severe exacerbation
or 2+ moderate exacerbation in one year
Asthmatic COPD treatment?
1) SABA or SAMA
2) LABA + ICS (Discontinue SAMA)
3) LAMA + LABA + ICS
Only if pt has severe exacerbation
Or 2+ moderate ones in a year)
Difference between non asthmatic and Asthmatic treatment?
Step 2 for non asthmatic is LABA + LAMA
Step 2 for asthmatic is LABA + ICS
*MAKES SENSE BECAUSE ICS IS MAIN TREATMENT FOR ASTHMA anyways
Which one continues throughout COPD treatments? SABA or SAMA?
SABA
In non asthmatic pt what to do if step 3 (triple therapy) doesn’t work?
Get rid of ICS
Go back to LABA and LAMA
Just like when ICS is trial in children under 5
How can u prevent COPD pt’s from developing something life threatening - HIGH RISK PT
Offer Pneumococcal & Influenza vaccine
If asthmatic and Step 3 triple therapy doesn’t work?
Add theophylline, Oxygen therapy, Mucolytics
Preventative ABX winter months?
Azithromycin 250mg TDS a week
Exacerbation in last year they need a Rescue pack, what’s part of it?
Antibiotic
(Amoxicillin/Doxycycline/Clarithromycin)
+
Oral Corticosteroid (Prednisolone)
