During LMFR what are the major steps of the Entry section (3.0)?
- Evaluate/ implement CTNMT closure- IF RCP on stop seal AND loss inventory notify maintenance to install shaft block- Stop core alts, place F/A in safe position- Monitor RCP trip criteria- Perform LMFR set- FLEX bro
During LMFR what are the major steps for a Dilution Accident?
- Stop all RCS additions- Chemistry sample boration source- Bypass LD IX- Eval dilution paths- LM-dilution flow path isolation if cant find source

- Level is less than expected or required for plant conditions
During LMFR what are indications of and actions for SDC cavitation?
SDC pump flow OR amps oscillations < 1 minute intervals- If 2 pumps in svc, stop 1- Reduce flow to 3780 gpm- Stop affected pump go back to HR success path- Ensure Inventory control is adequately addressed BEFORE leaving this path, it may hamper HR restoration
What are the level requirements to use SI sump recirc?
- CNTMT sump level ≥ 54”- RWT ≤ 9.4%
During LMFR when using HPSI for RCS make-up when would you use CL injection or HL injection?
CL injection:- SDC lost- Any CL manway removed- Nozzle dam NOT installed- HL not used, assumes water lost out HLHL Injection:- SDC lost- Any RCP impeller removed- RCP plug NOT installed- CL not used, RCP opening
What are the gravity feed paths for RCS make-up?
- SDC suction lines via LPSI suction- HL injection via HPSI piping- CL injection via HPSI/ LPSI/ CS piping
When using gravity feed for RCS make-up, what is the flow rate used?
- RWT level drops 3% every 15 minutes
What temperature/ pressure requirements would you isolate seal injection prior to starting CH pump?
- > 150°F- >150 psi
During LMFR what are the requirements to drain RFP deep ends back to the RWT?
- All F/A’s in the vessel or SFP- RWT available- Sump recirc not available or deireable- PC-118 closed
During LMFR, what are the requirements to use “Controlled Steaming” for pressure control? (Set 1 PC-4)
- Used in conjunction with HR-1 (forced circ) or HR-3 (nat. circ)- Lpzr ≥ 10% [15]- Lsg ≥ 10% [15]
During LMFR, what are the requirements if using RCGVS for pressure control (Set 1 PC-5)
- Lpzr <65% [64]- Vent to CNTMT preferred - A train to CNTMT (A to Air)- B train to RDT
