1 Glucocorticoids Flashcards
structure
identical to steroids produced by adrenal cortext
function
within cytoplasm rather than cell surface
indications
endocrine (not our focus)
- adrenal insufficiency
- physiologic doses
NON-ENDOCRINE
- inflammatory processes
- pharmacologic doses
glucocorticoids (names)
- Cortisone
- Solu-cortef (hydrocortisone)
- Depo-medrol (methylprednisolone)
- Prednisone
- Orapred (prednisolone)
- Triamcinolone
- Dexamethasone
Cortrosyn Stimulation test
Measures ability of adrenal cortext to produce
- adrenocorticotropic hormone (ACTH) administered
- degree of adrenal gland stimulation is measured
Prednisone + MOA
Glucocorticoid
Binds to/activates receptors in cytoplasm and nucleus
Suppression of leukocyte migration, decreased capillary permeability
Prednisone adverse effects
Osteoporosis Adrenal insufficiency Immunosuppression Psych stuff PPP (polydipsia, polyuria, polyphagia)
Prednisone considerations
DO NOT DISCONTINUE ABRUPTLY
AM dosing more physiologic (faster metab)
Growth and development effects
Inhibits cell division
Delays healing
Retards growth of children
Calcium metabolism effects
Increases bone resorption
Increases renal clearance
Suppresses bone formation
Decreased intestinal absorption of Ca and Vitamin D
Respiratory effects
Not bronchodilator
Inhaled = long term control, not immediate effect (>6 hours)
