TB 82 INFECTIOUS DISEASE PROTOCOLS

Rescue ambulance patient care areas and apparatus seats should be scrubbed with a 1:10 solution of bleach and water (1 part bleach to 10 parts water) when contaminated and then thoroughly wiped down with clear water and air-dried. DO NOT USE COMPRESSED AIR TO DRY.

LEVEL I - Contact limited to being in the presence of a suspected source patient (person suspected of having a communicable disease).

LEVEL II - Contamination of clothing or intact (non-broken) skin, by blood or body fluids.

LEVEL III - Exposure of non-intact (broken) skin or mucous/conjunctiva membranes to blood or body fluids. This category also includes needle punctures and human bites.

LEVEL I

No special action required for decontamination. Documentation of exposure to communicable disease in Station Journal and member’s personal record book (page 8A, F-745). See list of reportable communicable diseases, Manual of Operation, Volume 5, Section 9/7-00.00, and
F-420 instructions-Appendix 1.

LEVEL II

Decontamination of exposed clothing and body part. Documentation of exposure in Station Journal, member’s personal record book, and F-225 letterhead with member’s names and all details of exposure forwarded to the Medical Liaison Unit.

LEVEL III

  1. Decontamination of exposed body part.
  2. On-duty Captain notified at member’s assignment.
  3. Medical Liaison Unit notified in all cases (on-call officer after hours).
  4. EMS District Captain notified through MFC, in all cases. *
  5. First care sought, in all cases, at designated follow-up hospital. (Currently: Torrance Memorial, Good Samaritan, Holy Cross, and Cedars-Sinai.)
  6. F-420 completed and forwarded. Original left at hospital receiving the source patient, immediately.
  7. “Inquiry Requested” box to be marked on F-902M.
  8. Personal record book and station journal entries.
  9. F-166 and DWC Form 1 completed and forwarded.

EMS District Captain’s shall attempt to obtain source patient blood for testing in all Level III exposures.

Although there are numerous pathogens that may enter a host patient via the airborne route, the four most significant to our members, which will be covered in this document are severe acute respiratory syndrome (SARS), tuberculosis (TB), meningococcal meningitis, and smallpox.
