What are the diagnostic criteria for schizophrenia?

What drugs can cause psychotic symptoms?

What are your 1st generation antipsychotics?

A

Lack of emotion (apathy), social withdrawal, loss of motivation(avolition), lack of speech(alogia). Hallucinations, delusions, disorganized thinking/behavior.

Anticholinergics(centrally acting-high doses), cannabis , Dextromethorphan, Dopamine agonists(Requip, Mirapex, Sinemet), Interferons, Stimulants(especially if already at risk), Systemic Steroids(typically with lack of sleep, ICU psychosis), Bath salts, cocaine, lsd, meth, PCP.

Haloperidol, class is butyrophenone, Decanoate is monthly, IM.

What are the adverse effects of SGA’s?

What are the SGA’s?

A

Dementia-related psychosis, increased risk of death, Adasuve is bronchospasm, Sedation and EPS(lower potency has more sedation, higher potency has higher EPS), Dystonias (higher risk with younger males), centrally acting anticholinergics (diphenhydramine, benztropine). Akathisia (anticholinergics, benzodiazepines, or propranolol). Parkinsonism (anticholinergics or propranolol), TD is higher risk with elderly females, must stop and replace with SGA with low EPS risk, TD can be irreversible, IV haloperidol has high risk of CV effects, Adasuve also has dysgeusia, sedation.

Metabolic side effects(clozapine, olanzapine, quetiapine), EPS(lower risk quetiapine), Hematological effects(clozapine), qt prolongation(ziprasidone), increase prolactin (risperidone, paliperidone), seizure (clozapine)

Aripiprazole(abilify), Clozapine (clozaril), Lurasidone(latuda), Olanzapine (zyprexa, zydis ODT), Paliperidone (invega, sustenna), quetiapine (seroquel and seroquel XR), Risperidone (risperdal, risperdal consta).

What special things to know about Asenapine (saphris)?

What special things to know about Clozapine (clozaril)?

A

Abilify maintena (IM suspension give monthly), Aristada(IM suspension, give every 4-8 weeks). Akathisia, anxiety, insomnia.

Sublingual tablet, no food/drink 10 min after dose, tongue numbness.

Used no sooner than 3rd line, severe side effect potential, myocarditis and cardiomyopathy, seizures, weight gain, increased lipids and glucose, sialorrhea, agranulocytosis, REMS is pharmacies must be certified with clozapine REMS, ANC >1500, stop therapy if ANC <1000.

What to know about olanzapine (zyprexa, zydis ODT)?

What to know about paliperidone (invega, sustenna)?

A

Nearly weight, lipid and blood glucose neutral.

Relprevv, causes sedation and delirium, monitored for 3 hours post injection, somnolence, weight gain, increased lipids and glucose.

invega sustenna is monthly IM injection, invega trinza every 3 months(start only after receiving invega sustenna x 4 months), increased prolactin, EPS, especially at higher doses.

What to know about Risperidone (risperdal, risperdal consta)?

What to know about Ziprasidone (Geodon)?

A

Somnolence, orthostasis, weight gain, increased lipids, increased glucose. Take XR at night without food or with a light meal.

Risperdal Consta IM injection give every 2 weeks, EPS especially at higher doses, increased prolactin, weight gain, increased lipids, and glucose.

Take with food, Geodon IM, QT prolongation, CI’d with QT risk.

Can dantrolene be used in NMS?

A

Valbenazine(ingrezza), deutetrabenazine(austeda). Somnolence is valbenazine, hepatic impairment and somnolence.
