A

Elevated or irritable mood for at least a week or any duration if hospitalized. depression. At least 3 of: inflated self-esteem, sleeping less, talkative, jumping topics, easily distracted, increase in goal directed activity, involved with high risk pleasurable activities(need 4 if only irritable).

Bipolar 1 is severe mania, psychotic/delusional. Bipolar 2 is hypomania, not affect social, not cause psychosis, may seek help and be misdiagnosed as depression only.

Fetal valproate syndrome, cognitive effects, fetal carbamazepine syndrome, lithium, lamotrigine is safer while lurasidone is most favorable(bipolar depression is only approval).

What to know about lithium(lithobid)?

What are the lithium drug reactions?

A

0.6-1.2 mEq/L trough level. GI upset, cog effects, cogwheel rigidity, tremor, weight gain, polyuria/polydipsia, hypothyroidism. >1.5 is coarse hand tremor, etc >2.5 is more. Serum lithium levels, renal function, thyroid function. Lithium is 100% renally cleared, avoid use with other serotonergic agents.

increases with lower salt intake, ace inhibitors, arbs, nsaids. Decreases with increased salt intake, caffeine, and theophylline. Increased risk of serotonin syndrome if taken with lithium, SSRI, SNRI, triptans, linezolid, and other serotonergic drugs.
