Dyslipidemia Flashcards
What are your statin equivalency doses?
What are the Statin’s warnings?
What are the statin’s brand names and which one do you need to take in the evening?
Pita-2, Rosu- 5, Ator- 10, Sim-20, Lov-40, Prava-40, Flu-80. Pharmacists Rock AT Saving Lives and PReventing Flu
Acute liver disease, pregnancy, breastfeeding, strong CYP3A4 inhibitors(simva and lova). Skeletel muscle effects, niacin, hepatotoxicity, arthralgias, myopathy, get lft’s at baseline and lipid panel every 4-12 weeks.
Atorvastatin- Lipitor, Pravastatin- Pravachol, Rosuvastatin- Crestor, Simvastatin- Zocor. Take Zocor in the evening. Lovastatin IM is taken with evening meal.
What are the statin drug interactions?
What are the big things to know about ezetimibe?
What to know about bile acid sequestrants?
Avoid cyclosporine with atorvastatin. Simvastatin Max 10(verapamil, dilti, dronedarone), Max 20( amio, amlo, lomitapide, ranolazine). Lova Max 20-40, all listed plus danazol and ticagrelor. Colchicine has also shown to have myopathy when coadministered.
inhibits absorption in the small intestine. Watch for myalgias. Brand name zetia.
Colveselam(welchol) CI’d in bowel obstruction, constipation, ab pain, cramping, bloating, gas Colesevelam can be used in a pregnant patient. Take 1-4 hours before or 4-6 hours after all other drugs when using cholestyramine and colestipol. Can lower ADEK(fat soluble vitamin) absorption.
How do the fibrates work?
What are the fibrates side effects?
What are the fibrates brand names?
decrease TG significantly, catabolism of VLDL and increased lipoprotein lipase activity, PPARa activators.
Take fenoglide and lipofen with meals. Severe liver disease including primary biliary cirrhosis and gall bladder disease CI’d, watch for myopathy(increased when co administered with a statin). Dyspepsia, Can increase ldl when TG are high. May increase effects of sulfonylureas and warfarin, Gemfibrozil should not be given with ezetimibe or statins.
Antara, Tricor, Trilipix. Lopid(gemfibrozil).
What to know about niacins moa?
What are the side effects of niacin?
Can lovaza increase LDL?
Decreases synthesis of VLDL(decreased TG) and LDL
Rhabdomyolisis, hepatotoxicity, watch for flushing and itching, N/V/D, increased bg and hyperuricemia(or gout), check LFT’s, CR has less flushing but more hepatotoxicity, Niaspan is best choice, take with aspiring 325 before the dose, take with food, avoid spicy food, they are not interchangeable, can increase HDL 15-35%. take 4-6 hours after bile acid sequestrants.
YES. Can only take fish oils when TG >500
What to know about lomitapide?
Boxed warning of hepatotoxicity, CI’d in acute liver disease and pregnancy. Juxtapid has a REMS program, Kynamro(mipomersen) does as well. used in HoFH
