How do you treat inpatient CAP?

A

Amoxicillin: 80-90 mg/kg/day or Augmentin 90 mg/kg/day. Ceftriaxone IM or IV given for NPO or vomiting patients.

Augmentin.

Beta-lactam(ceftriaxone or cefotaxime) + macrolide(azithromycin) or doxycycline. Or Respiratory quinolone monotherapy.

How do you treat active TB(RIPE)?

A

No antibiotic use in last 3 months is macrolide or doxycycline. If risk for MRSA Beta-lactam +macrolide or doxycycline. Respiratory quinolone.

Isonazid for 9 months(preferred for HIVE patients, pregnant women and children). Rifampin for 4 months or INH and Rifapentine once weekly for 12 weeks.

Rifampin,Isonazid,Pyrazinamide, Ethambutol for 2 months. Scale back to 2 drugs for 4 months after that INH and RIF daily or 3x per week.

YES.

Hepatitis, watch for peripheral neuropathy(pyridoxine decreases this risk), watch for hemolytic anemia and increased LFT’s.

Optic neuritis(dose related), increased LFT’s.

Bactrim and Doxycycline.

Nitrofurantoin. Bactrim if no sulfa allergy.

If local resistance < 10% it’s cipro or levo, if higher it’s ceftriaxone, bactrim, beta lactam.
